By the time they hit 50, about half of the guys I see have dealt with male pattern hair loss. It isn't some medical mystery; it's mostly DHT (dihydrotestosterone) shrinking your follicles down until they give up. Look—the science is actually on your side here, provided you start before you’re trying to grow hair on a bowling ball. Most of what we use is cheap, effective, and heavily researched. [1]

Finasteride: the foundation

Think of finasteride as the floor of your treatment. It stops testosterone from turning into DHT, which is the main culprit behind that receding hairline. After a year on it, 90% of men either stop losing hair entirely or see some regrowth. I know everyone worries about the sexual side effects, but frankly, they only show up in about 1–2% of patients in clinical trials—and for the vast majority, things go back to normal if you stop the med. Minoxidil: the regrowth lever

While finasteride protects, minoxidil grows. It works by opening up blood vessels in the scalp and forcing your hair to stay in the growth phase longer. While the 5% topical liquid is the old standby, I find a lot of folks can't stand the grease; that's why we often pivot to low-dose oral minoxidil (2.5–5 mg). It’s cleaner, easier to remember, and honestly, a lot of my patients prefer it over the topical mess. [3]

The modern stack

  • Standard 1 mg finasteride daily (though we can go topical if you’re particularly sensitive)
  • Daily minoxidil, either in a pill or a foam
  • Microneedling every week or two to wake up the follicles and help topicals sink in
  • GHK-Cu copper peptides to keep the scalp environment healthy
  • Dutasteride—this is for the guys who don't see results with finasteride alone

The honest timeline

I’ll be straight with you: you might actually lose a little more hair between weeks 4 and 8. It’s called a 'dread shed' and it just means the follicles are clearing out old junk to start new growth. You won't see real thickening until at least month 4, and the full 'wow' factor takes a year. Just remember—this is maintenance, so if you stop, the gains will vanish within 6 to 12 months. [1]

Who should skip this

This isn't for everyone. Finasteride is a hard 'no' for women who could become pregnant, and we tread carefully if there's a history of male breast cancer or severe, untreated depression.
